Fassity Home Care LLC is a distinctive, accepting organization of dedicated staff that provides twenty-four (24) hours of non-skilled services, Skilled services, private duty nursing, and In-Home Support services to our clients in need. We are state-licensed and Bonded professionals who provide home health care and in-home supportive services to clients. We take pride in creating a warm and inclusive environment where you or your loved ones feel safe, comfortable, and valued.
We strive to grant you customized skilled care, non-skilled care, and in-home support services for your physical health needs. On top of that, we have a compassionate staff that will emotionally support you through your health crises.
When at home, the client gets dedicated attention from professionals and still manages to be comfortable in the state they are in, simply because they are at home. In reality, home health care services can give patients more ease and serenity compared to institutional forms of care.
Fassity Home Care, LLC can provide a combination of services in your home – all under the direction of a trained physician and the interdisciplinary team (IDT). Working with your doctors, Support Coordinators, Care Coordinators/interdisciplinary team, our qualified staff will:
- Plan, coordinate, and provide personalized care to all clients – unique care for unique individuals
- Provide person-centered care and support on the unique strengths and abilities of your loved ones
- Affordable and cost-effective care services
- All our services are geared towards assisting individuals to be as healthy and independent as possible
At Fassity Home Care LLC, Healthcare services can be provided in:

- An independent living facility
- An assisted living facility
- There may be situations where the client is cared for in school and hospital.
At Fassity Home Care LLC, In-Home Support services can be provided in:
- Client’s home
- Family home
- Community settings
